Hello doctor,
I have had severe rosacea for the past few years, even though I never had it during my teens. I initially thought it was due to overusing retinoids and irritation, but after stopping retinoids for over a year, my skin remains consistently red and hot. Despite using moisturizers, my skin still feels dry and tight. I suspect Demodex mites might be contributing to my rosacea. Since topical Ivermectin is not available in my country, would it be okay to use Permethrin on my face to treat Demodex?

Avoid self-medication. Start by washing your face with a gentle cleanser twice daily for two weeks. Then, apply Metronidazole one percent gel (MetroGel) during the day for two weeks. For added treatment, use Benzac AC 2.5 percent cream (Benzoyl peroxide) on alternate days for two weeks.
In addition, it is important to avoid known rosacea triggers and drink plenty of water to keep your skin hydrated. Use sunscreen with SPF 30 for 30 minutes before going outdoors to protect your skin from the sun. Please attach a clinical image to help with further guidance.
