How to manage the red rashes and patches on the skin affected by psoriasis medications?

Hello doctor,

My concern is I have psoriasis and have been under treatment for almost a year now. I actually mistakenly put psoriasis ointments on some healthy parts of my hands, and now it irritates that part of the skin by having rashes and red patches, and also, the skin quality of the healthy part has too much deteriorated. My dermatologist suggested using a skin moisturizer on those parts, and I have tried. However, it is not getting back to the normal skin I had. Please advise me on what I could do.

I understand your concern.

As per the history and image provided (attachment removed to protect the patient's identity), red lesions or irritant dermatitis, or exfoliative dermatitis, appears due to skin atrophy which is skin thinning and hypersensitivity due to the application of exfoliating or strong medications. Psoriasis medication, including steroids and salicylic acid, which are commonly prescribed for the treatment of psoriasis, lead to skin thinning when used in excess or when used on normal skin leading to redness, irritation, burning sensation, and discomfort due to disruption of the protective barrier that skin provides.

I suggest you follow these:

1. Oilatum emollient (Liquid paraffin) or bath formula two caps full in one liter of water may be used for washing the affected area once daily. They provide protection and hydration, preventing secondary bacterial infections. Kindly refrain from using exfoliants or any skin care products, including soaps, scrubs, or shower gels, for four weeks or till the rashes heal.

2. In case of pain or tenderness on touch, Fusidic acid ointments alone may be used on the affected area once daily at night.

3. Continue using just water for cleansing the affected area for four weeks, except when using Oilatum mixed with water once daily.

4. Tablet Ascorbic acid 500 milligrams chewable tablet once daily after food at night for one month helps with faster healing.

5. Capsule Evion 400 milligrams (vitamin E capsule 400 milligrams) may be used in the morning after food for one month. They help with keeping skin hydrated.

6. Novasoft cream (Liquid paraffin and White soft paraffin) may be applied in a thin layer on the affected areas to provide hydration and protection, which also helps in faster healing of the skin twice daily in the morning and at night.

7. Avoid oily or fried food, dairy products, and sweets. And make sure to consume more fruits and vegetables and at least 12 glasses of water daily.

8. Cold compresses may also be applied to the affected area twice daily for a soothing effect.

9. Capsule Omega 3 once daily after food at night for one month helps with hydration and faster healing and augmenting the effect along with Evion capsules.

As I have mentioned, skin thinning causes hypersensitivity, due to which they appear as inflamed, red lesions, skin repair takes at least 40 days, and therefore it is suggested to be consistent with the medications and avoid skin care products on the affected area to help with faster healing. As your doctor has mentioned, Vaseline (oat extract and soyabean oil) alone would also suffice as they help with hydration. However, the above medication will help with quicker healing and skin protection from secondary bacterial infections as well, which may be common due to the loss of mechanical barrier that skin provides.

Do not worry, and they may be managed with the above medication. However, it may take a few weeks for complete recovery.
