Hello Doctor,
I have been experiencing itching in the penile and anal areas for the past two to three months. I have been taking Cefixime 200 milligrams, which provides relief for a few days. Urine examination shows the presence of pus cells. However, after a few days, intense itching returns in the penile area, and I also experience itching in the ears.
I have been taking Montelukast and Levocetirizine combination tablets for the past seven years. I would like to know the possible cause of these symptoms. I suspect reduced immunity. Which tests can confirm the cause? I recently underwent testing for herpes simplex virus (HSV), which was negative. Symptoms worsen after ejaculation during nightfall. I have never had sexual intercourse. Images of the affected area are attached.

Itching in the penile and anal regions is very commonly due to a fungal infection. These areas remain moist and are prone to fungal overgrowth. Do you notice any rashes, circular patches, whitish discoloration, or scaling in these areas? Are you diabetic?
Regarding immunity, you may undergo a complete blood count (CBC) and erythrocyte sedimentation rate (ESR).
I would suggest the following prevention measures that include:

Please upload clear images of the affected areas showing the lesions. A potassium hydroxide (KOH) scraping test can be done to confirm a fungal infection.

In such a case, it is unlikely to be herpes. Herpes usually resolves by itself in five to seven days.
The most probable conditions could be:
- Milia.
- Sebaceous hyperplasia.
- Fordyce spots.
All three are benign. They are just due to increased size or activity of the oil glands or their blockage.
