Hello doctor,

I have been suffering from a few symptoms, such as involuntary passing of seminal fluid with urine (urine is cloudy), the passing of semen with bowel movements, and premature ejaculation of semen for the last five years. I have my kidney, prostrate scans performed, and had tests for sexually transmitted disease (STI). I have tried herbal remedies and abstinence from masturbation and pornographic material with no benefit. I am otherwise perfectly healthy. I am 24 years old, 6.2 feet tall and weigh 180 pounds.

In my opinion, you might be suffering from premature ejaculation. It is early ejaculation with a latency time of fewer than 1.5 minutes. Medical treatment is mostly by selective serotonin reuptake inhibitors (SSRI). I suggest tablet Lejam (Dapoxetine) available in two dosage forms (20 mg and 40 mg) once daily. Start with 20 mg, and if the symptoms persist, switch over to 40 mg tablet. Consult your specialist doctor and discuss with him or her and take the medicines with their consult.

Usually, the treatment for premature ejaculation depends on the severity of the disease. Since you suffer from frequent ejaculation, I have suggested the medicine to prevent unwanted ejaculation. You can also incorporate a few behavioral changes in your daily life, such as interrupting the intercourse whenever you have the sensation of ejaculation continue afterward. Ejaculate one hour before the sex to prevent ejaculation sensation during the intercourse. Apply a local anesthetic spray like Xylocaine (Lidocaine) on the penis before having sex. Vigorous exercise and regular checking of testosterone levels will also help increase sexual strength. All these measures will help improve the control over the premature ejaculation sensation with time. Medications do not cure the disease perse but will help control the symptoms; their dosage can be reduced when you get adapted to the new lifestyle.
