Hi doctor,

I have been referred to go see a urologist as I have a small quantity of free fluid in the pouch of Douglas and beside the right ovary. I have this feeling for about three months. I always feel like I have a full bladder and sometimes get pain. Sometimes I have constipation and cramping-like pain after eating. The bladder is well distended and thin-walled. The postvoid residue of 0.07 pints. Is this serious? Kindly give your opinion.

Fluid in the pouch of Douglas can be a physiological thing and can be seen after ovulation in the mensural cycle. If you have urgency, frequency of micturation, then you might be having an overactive bladder. I suggest you take tablet Mirabegron 25 mg once in the afternoon for one month. We will wait for how you respond to this medication before considering further investigation or treatment plan.

Will this help the feeling of always having the feeling of fullness? I was just when I got the scan. It was two weeks before my period. I have two different consultants, a male, and a female. Does it matter if it is a man or woman as I am female? Would a woman be better at diagnosing and understanding me, or does it matter? How to get a prescription? Kindly give your opinion.
