Hello doctor,

I was diagnosed with post-strep glomerulonephritis, which is now clearing up. However, I believe this will happen again. Last month, my brother developed a sore throat, chills, and fever. And four days later, I had oral sex. I developed a sore throat that same night. The following day, I decided to get a medical consultation. I was diagnosed with tonsillopharyngitis and given Azithromycin 500 mg orally (once a day) for six days. I immediately began the medication. However, the same night after taking the medicines, I experienced a right lumbar abdomen ache. I also had a small burn while urinating. I did not observe any changes in urine color. I also have a low backache. The next day, I had to take a medical examination for a job. The results were microscopic hematuria: 31 RBC/HPF 4 WBC per microliter. I was instructed to have a repeat urinalysis with RBC morphology, and the results showed a WBC of 1 to 2 per microliter and an RBC of 12 to 15 per microliter, with the majority being dysmorphic. I was directed to a nephrologist, who ordered a repeat urinalysis with RBC morphology, a KUB ultrasound, a BUN and creatinine check, and an ASO test. The results were: RBC: 1 to 2 per microliter, no WBC; RBC) morphology. The majority of RBC were dysmorphic, an unremarkable KUB, normal bun and creatinine levels, and a positive also test. The nephrologist observed that I had post-strep glomerulonephritis, which had resolved. However, my family still had a throat infection when I returned home. I think I got it again. Also, I had oral sex again today. Kindly guide me.

PSGN (Post-streptococcal glomerulonephritis), also known as post-streptococcal glomerulonephritis, develops only after the throat infection has cleared, meaning you may develop it after three weeks after having a throat infection, and there is a possibility of recurrence if not treated properly.

I suggest drinking hot water and staying hydrated. Make sure you are passing good urine. Maintain a regular blood pressure monitoring schedule. If it is high, it needs treatment. If your feet are swollen, they should be addressed. Otherwise, you do not require extra medication. I want you to repeat the blood test for ASO titer (anti-streptolysin O titer test), urine analysis, and RBC (red blood cell) casts. Do not take any more antibiotics. Use a betadine gargle two or three times per day. Avoid oral sex for a while. PSGN is a very mild condition in which the disease rarely progresses to a chronic stage requiring dialysis (process of removing excess water, solutes, and toxins from the blood in people whose kidneys can no longer perform these functions naturally), although care must be taken.
