What could cause unusual bleeding after withdrawal bleeding?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I took Meprate to delay my periods for a few days a month back. But I had a spotting before three days onwards. After that, I got withdrawal bleeding on the stoppage of Meprate after fifteen days. Now, I have had slight bleeding for two days. I am having PCOD. What does this bleeding mean? Can it be implantation bleeding? Is there any risk of pregnancy if I have unprotected sex in between these days?

Please suggest.

Hello,

Welcome to icliniq.com.

Since you have PCOS (polycystic ovarian syndrome) and recently took Meprate (Medroxyprogesterone acetate) to delay your period, the bleeding you are experiencing could be due to hormonal fluctuations rather than implantation. Let us break it down.

Spotting for three days before taking Meprate - This could have been an irregular bleed due to PCOS or pre-period spotting.

Withdrawal bleeding for five days - After stopping Meprate, withdrawal bleeding is expected. This is not a true period but a response to the drop in progesterone.

Bleeding for two days - This could be an early period, irregular breakthrough bleeding due to hormonal imbalance, or ovulation spotting if your cycle is trying to regulate itself. Implantation bleeding is usually light pink or brown and lasts for one to two days. However, it is rare in PCOS due to irregular ovulation.

Risk of pregnancy -

Pregnancy is possible if you have unprotected sex around ovulation. Since your cycle is irregular due to PCOS and medication, it is hard to predict when you will ovulate. If you had unprotected sex anytime after stopping Meprate, there is a chance of pregnancy, though it is lower in PCOS.

What you can do -

Take a pregnancy test now, and if negative, repeat it after three to five days to confirm. If the cycles remain irregular, your hormones might still be adjusting after stopping Meprate. If pregnancy is not the goal, consider using protection consistently, as PCOS can cause unexpected ovulation.

I hope this helps.

Thank you and take care.
