Hi doctor,

I am in my mid-thirties. I had unprotected sex with a non-sex worker on the third day of the last month. On the seventh day, I got the tests (STI-9 panel) done for syphilis and HIV (human immunodeficiency virus).

Both of them came out to be negative. I got the test (HIV fourth generation) done on the 25th day of last month, and again the result was negative. However, I have post-nasal dripping, especially at night, and I have lost one pound of body weight recently. I have no other symptoms.

My partner also got the tests done on the seventh day of the last month, and the results were negative. I am paranoid, so although COVID-19 and influenza are widespread, I want to know my status. May I get some expert advice?

Thank you for the query. I understand your concern. The chances of transmission of HIV (human immunodeficiency virus) by single unprotected intercourse with an HIV-positive person are low.

If she is negative, then there is no chance of transmission. It is good that your partner's HIV test is negative, but this does not rule out the HIV status completely. Your HIV antibody test is also negative after around 22 days of exposure, which is good, but it is not conclusive.

The chances of an HIV test coming positive later are very low. Still, it would be better to go for an HIV antibody

test after three months of exposure to have conclusive results and to be relieved of anxiety. The symptoms are non-specific and can be due to any respiratory infection.

I consulted you two months back, but I still have a concern. Although you mentioned low risk and do tests again after three months (next month).

I am concerned and did the test again because I had a backache, wet cough, stuffy nose, and occasional sneezing. I also live at high altitudes where it is cold, mainly in the evening and at night. Also, I will do the test again in the third month. Thank you for your time, doctor.

Sorry to be a bother. So I have experienced a sore throat, for which I recovered, but I lost weight, and some section of the tongue looks white, but he said it could not be scraped off, and no bleeding occurred, and he does not think it is oral candidiasis.

If you contact a person with syphilis, it would be better to consult your doctor for further examination and management.

Syphilis tests like RPR (rapid plasma reagin test) and treponemal tests may be needed, and presumptive treatment for syphilis, like Penicillin, may be given after counseling and discussions.
