Which pneumococcal vaccine is best for a 78-year-old?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My grandfather is 78 and recently got the pneumococcal vaccine. Are there other vaccines, like Prevnar 20, that could help protect him even more? Also, because of his age, are there any signs of pneumococcal pneumonia we should watch for?

Kindly guide.

Thank you.

Hello,

Welcome to icliniq.com.

I read your query and understood your concern.

Vaccines for better protection in older adults include the following:

  1. Pneumococcal vaccines: Prevnar 20 (PCV20) is one of the latest pneumococcal vaccines, providing protection against 20 serotypes of pneumococcus. If your grandfather has already received this, no additional pneumococcal vaccination is needed. The other one is PPSV23 (Pneumovax 23). If your grandfather received an older pneumococcal vaccine (like PCV13), he may still need PPSV23 for broader protection. Check with his doctor about the timing and need for additional doses.

  2. Influenza vaccine: A yearly flu shot is important because influenza can make older adults more vulnerable to complications like pneumonia.

  3. COVID-19 vaccine: Make sure your grandfather is up to date on his COVID-19 vaccines and boosters, since respiratory infections like COVID-19 can worsen lung issues.

  4. Shingles vaccine: The recombinant zoster vaccine protects against shingles and associated complications, which is also important for older adults.

  5. Tdap or Td booster: This vaccine protects against tetanus, diphtheria, and pertussis. A booster is recommended every ten years.

Pneumococcal pneumonia can be harder to detect in older adults. Watch for signs like shortness of breath, persistent cough with green or yellow mucus, fever or chills, fatigue, chest pain, confusion or forgetfulness, and poor appetite. If these symptoms appear, especially after a respiratory illness, contact a doctor right away.

If any of these symptoms develop, especially after exposure to respiratory illnesses, seek medical attention promptly.

I hope this has helped you.

Kindly follow up if you have more doubts.

Thank you.
