Hi doctor,

I am a 50-year-old male. I am diabetic for the past ten years. I have had severe foot pain for the past 15 days. The pain in my left foot is more severe compared to my right foot. The pain is like pins and needles and is also intermittent. This foot pain often occurs at night. My doctor diagnosed my condition as diabetic peripheral neuropathy. My doctor advised me to take Duloxetine, Pregabalin, Nortriptyline, and Methylcobalamin 1500 mcg. I also take Quitipin for depression. Am I on the right track? I need a second opinion on my condition. How to manage diabetic peripheral neuropathy?

Diabetic peripheral neuropathy is not a curable condition. With available medications, symptoms of pins and needle sensation can get improved. If you have doubts about diagnosis, then I would like to know some further details about your symptoms,

1. Did the foot pain started since 15 days? Or it has been before also, but increased in intensity since 15 days? If so, I would like to know since these symptoms started?

2. Are these pins and needle sensation or pain is limited to feet? Or it involve above calf and thigh too?

3. Is there any other kind of pain in leg (thigh or lower leg)?

4. Do you have any lower back pain or neck pain?

5. Do you have urine control difficulty?

6. Have you undergone any investigation for these symptoms, like nerve conduction study or MRI (magnetic resonance imaging) spine or blood work? If yes, please send the reports as attachments.

7. How was your symptoms of pins and needle sensation on mentioned medications?
