Is it normal to have a reduced flow on the third day of periods after three months of no menstruation?

Hi doctor,

I have been a PCOS/PMOS patient for the past eight years. I had my last period three months back as I have withdrawn my tablet Medroxyprogesterone. After these periods, I have not got any periods in the last three months and was not even sexually active. I have recently got sexually active this month but with protection. Then, I consulted my doctor, and she again recommended the tablet Medroxyprogesterone for seven days. I started taking the medicine, and after four to five days, I had cramps, after five days, I had faint bleeding. On the first day of the period, I passed normal blood clots and red cervical mucus while urinating, as I usually get during my periods. But now, on my third day, I have not got any period flow, and also my red cervical flow also reduced. I had my last to last period five months back. And currently, I am taking a tablet of Femilon from the day of my first period as prescribed by my doctor. So, doctor, I am a little scared. Is it pregnancy? Please help.

I understand your concern. I recommend you exercise and make lifestyle changes, precautionshaving some lifestyle changes, precautions, investigations, and medications. I suggest the following:

  1. Daily walk and exercise to obtain a normal BMI (body mass index) ratio and weight.
  2. Have a healthy diet with home-cooked food with less salt, oil, spice, and sugar.
  3. Avoid recreational drugs and go for recreational activities.
  4. Check for any health issue you have and try to have regular, complete treatment for that.

Apart from these, you have a hormonal imbalance. So, you have a weight gain, and your periods are not regular. You have already started with an oral contraceptive pill, so continue it for three months to have hormonal support, regulate cycles, and monthy menstruation. And you have already started with tablet OCP Femilon so continue it for three months to have hormonal support and regulation cycles and to have menses monthly. For investigation, I will suggest you have a transvaginal scan. And on the second day of the period, go for a blood test that should include serum progesterone, serum testosterone, serum insulin, serum prolactin, FSH (follicle stimulating hormone)/LH (luteinizing hormone), and thyroid function test. After getting the laboratory test reports, you can discuss them with your gynecologist or me to get treatment accordingly.
