Can PCOS/PMOS cause a delay in periods?

Hello doctor,

I am a 29-year-old female with PCOS/PMOS. My period cycle is 34 days, which is rarely irregular once or twice a year. I have been tracking my ovulation days on the Flo app. I have not gotten my period for the last three weeks.

So, I did a home pregnancy test and saw a faint line. The doctor has prescribed me the tablet Duphaston for five days, but I have no periods and am on the 69th day of my cycle. I had brown discharge for about five days, but did not have any discharge after that. I also have back pain. What should I do?

I suggest the following instructions, especially if you have a history of PCOS (polycystic ovary syndrome)/PMOS (polyendocrine metabolic ovarian syndrome), as it can commonly cause irregular periods and hormonal imbalance:

  1. Take a serum beta hCG (human chorionic gonadotropin) test to know your pregnancy status.
  2. If the beta hCG is negative, I suggest taking combined oral contraceptive pills for three months to help regulate your cycle and improve the bleeding pattern, which is often recommended in PCOS/PMOS-related irregular periods.
  3. If the beta hCG is positive, repeat the test after 48 hours to assess the status of the pregnancy, and a transvaginal scan should be done to confirm the pregnancy.

I also suggest the following lifestyle measures, which are beneficial for PCOS/PMOS management as well:

  1. Exercise regularly to maintain an ideal body weight.
  2. Eat a healthy, balanced diet.
