Hello doctor,

I recently went to the ER after getting into a physical altercation with someone. I was winded and experienced chest and back pain. The pain eased while I was in the ER waiting for blood work and other tests. An ECG showed a septal infarct, which cannot be ruled out. I also had an echocardiogram two years ago that was normal. I experience palpitations during times of significant stress and have been diagnosed with anxiety and moderately high blood pressure. Currently, I am taking Levothyroxine for hypothyroidism.

I have reviewed your ECG (electrocardiogram) (attachment removed to protect the patient's identity). There are no definite changes in the ECG that suggest an acute major heart attack. Even an anteroseptal infarction does not meet the criteria. However, considering your symptoms, the most likely possibility is a minor heart attack known as unstable angina, which raises the concern for blockages in your arteries. Were troponin or CK-MB (creatine kinase) levels tested? Have you been prescribed any medications, such as Aspirin or statins?

I recommend that you start these medications and get further evaluation. Consult your specialist doctor, discuss with him or her, and take the medicines with take the medicines with their consent. In the meantime, maintain a healthy lifestyle and avoid smoking and strenuous activities.
