What causes penile discomfort after the second round of sex?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I experience pain after waiting for a second round during sex. Is that normal? I did some research but still have no answer to this. Should I maybe wait for more hours before having sex again?

I have no issues with my erection. The pain is felt on the sides of the shaft, from the base to below the head, and it appears when stimulation stops and after ejaculation of the second round. Is this normal?

The second one is enjoyable but annoying; I wash it, and it starts to disappear when the erection goes down. I just want to make sure it has nothing to do with scar tissue or something.

Kindly help.

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

I do not think it is anything to worry about, although we must rule out prostatitis (for which I am going to ask you some questions):

  1. Does the pain happen when you ejaculate during your first round of sex, or is it only during the second round?

  2. Do you have diabetes or hypertension?

  3. Do you have any pain when you pass urine?

  4. How long do you wait in between before you try to have sex again?

  5. Have you ever had any injuries to the penis area?

  6. Roughly how long have you had this issue?

  7. Was there any specific trigger before the pain started, or did this start happening randomly?

  8. Does this pain also happen on the occasions when you ejaculate by masturbation?

  9. Does your job involve you sitting down for prolonged periods in a stretch?

Revert with the answers to assist further.

Thank you, and take care.

Regards.

Patient's Query

Hello doctor,

Thank you for your reply.

No, it is the second round, and most importantly, the pain is not during ejaculation. Ejaculation is fine, but the physical feel of the shaft changes when the stimulation stops or after ejaculation until the erection goes away. It is felt on the sides of the shaft from the base to below the glans.

No, I do not have diabetes or hypertension.

No pain when passing urine; however, there is a slight pain when touching the area below the urethra between the tip and frenulum, and sometimes it is extended to the middle of the shaft (the urethra way). This feeling of pain when touching the skin; I can give it three to four out of 10. But the urine is fine.

I do not calculate, but I wait 30 minutes to an hour, sometimes more; it depends, but not right away.

I had no injury, but in the past two years ago, I was given an erection testing injection before marriage by a doctor, which forced an erection. The erection lasted for about five hours. However, the doctor confirmed that there was nothing to worry about after I talked to him; it did not go away, and he asked me to put it in ice packs. I do not know if this caused any minor injury, but the doctor confirmed that there was nothing to worry about. Later, the erection is fine.

I cannot answer this question because not all the time I do second rounds; I think this is random. I do not think there is a specific trigger. Sources online say this is normal and overstimulation. Other sources say it could be a minor injury. The last time, I gave myself about 10 days with no sex, but it just happened, so it does not make sense there is overstimulation from the first round.

I never tried a second round of masturbation, but it will be the same as sex. I feel it can happen again with masturbation.

No, most of the job work is outside, not sitting down.

Last thing, I did some research about P-shot, but I do not know if this will help. Also, I am not sure if it is normal for people to feel this on the second round due to age, if it is something related to the injection in the past, or if the tissue needs some time to rest so that the second round feels fine. I do not know. I am active in the gym.

Kindly help.

Hello,

Welcome back to icliniq.com.

I read your query and can understand your concern.

I do not think this is because of an injury; if that were the case, it would happen even during the first time.

I do not think this is prostatitis because the typical pain for prostatitis is generally immediately after ejaculation (and it would happen after the first time, too).

I do not think it is because of the injection, either. The effects would not last two years, and besides, you claim to have normal erections now.

P-shot (platelet-rich plasma) is not for you; it is for men with erectile dysfunction. Besides, there is very little concrete evidence to suggest its use.

I do believe this is just normal for age or maybe over-jealous intercourse.

I could advise you to test for prostatitis, but I do not suspect we will find anything, seeing as you are quite active and have regular sex.

It would be interesting to see if this happens during masturbation. For now, I have only reassurance to give you and maybe advice, and you avoid the second round for a month or so.

I hope I have answered your question.

Let me know if I can assist you further.

Regards.
