Can norovirus infection be avoided in healthcare settings?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello, Doctor,

My mother works in a nursing home where four patients are currently infected with norovirus. Although she is feeling fine, we are worried she might bring the infection home.

  1. Is there any way to prevent catching it?
  2. Should she wear a mask or gloves while caring for the patients?
  3. How long does the virus survive on surfaces like clothing or bedsheets?
  4. Would that be enough if she changed clothes after work and washed her hands thoroughly?
  5. Are there any cleaning agents you recommend for use at home?

We have a child with asthma at home, so I am particularly concerned about the risk.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I understand your concern.

To prevent the spread of norovirus, it is important to take extra precautions, especially considering the risk to your child with asthma.

Here are some recommendations:

1. Precautions at work: Your mother should wear a mask, gloves, and possibly a gown when handling patients who are infected with norovirus, especially since the virus spreads through direct contact with contaminated surfaces and fluids. Frequent handwashing is crucial, and she should avoid touching her face after patient contact.

2. Personal hygiene at home: After work, your mother should immediately change clothes and wash them. She needs to wash her hands thoroughly, and if possible, shower. This helps reduce the chance of bringing the virus into the home.

3. Cleaning surfaces: Norovirus can stay on surfaces for days, so it is important to disinfect commonly touched surfaces like doorknobs, light switches, and countertops. Use an EPA-registered disinfectant effective against norovirus or a bleach solution (1/4 cup of bleach in 1 gallon of water). Laundry items like sheets and clothes should be washed with hot water and detergent.

4. Home hygiene: In addition to disinfecting surfaces, regularly clean hands with soap and water or use hand sanitizer, especially after handling anything potentially contaminated.

5. Precautions for children: Be especially vigilant around your child with asthma. Avoid close contact with any potentially contaminated surfaces or people until proper hygiene is maintained.

I hope this helps.

Kindly follow up if you have more concerns.

Thank you.

The Probable causes

The most likely cause of concern is the potential exposure to norovirus at the workplace, where multiple patients are infected.

Investigations to be done

There is no immediate need for further Investigations unless the mother or child shows symptoms of norovirus infection. Monitoring for any respiratory issues in the child due to asthma exacerbation is important.

Differential diagnosis

Respiratory infections, particularly with asthma exacerbation, in children. Gastrointestinal symptoms like diarrhea and vomiting in the patient or child could also be attributed to norovirus.

Probable diagnosis

Exposure to norovirus in the nursing home. No current infection, as the mother is asymptomatic but at risk of being a carrier.

Preventive measures

Strict hygiene practices at home and work. Encourage the child with asthma to maintain their usual treatment and keep track of any respiratory symptoms. Clean surfaces regularly with bleach or a disinfectant known to kill norovirus.

Regarding follow up

Monitor the mother and child for any symptoms of norovirus infection, particularly gastrointestinal symptoms in the mother or respiratory issues in the child. If symptoms appear, especially if the child shows worsening asthma symptoms, contact healthcare providers immediately.
