I am a 47-year-old female and have been living with multiple sclerosis for several years. Now that I am completely paralyzed, I am finding it increasingly difficult to cope, both physically and emotionally. Despite being on several medications, I am experiencing constant muscle stiffness, spasms, and pain, which make it hard to stay comfortable, even when lying down. I am also dealing with frequent infections and other complications, such as pressure sores, which seem to worsen despite taking precautions. Fatigue is overwhelming, and I feel entirely drained most of the time.
I worry about the long-term effects of my condition and whether my current treatment plan is enough to manage these issues. Are there other therapies, medications, or interventions that might help improve my quality of life? Should we explore options like advanced assistive technologies or changes to my care routine?
I am also struggling emotionally with feelings of frustration and helplessness. What can I do to maintain some sense of control over my health and well-being, given my condition?

This is an incredibly tough situation, and it is understandable to feel both physically exhausted and emotionally overwhelmed. Managing advanced multiple sclerosis (MS) requires a comprehensive approach beyond just medications.
Your current medications might not be fully controlling your symptoms, and dose adjustments or introducing new medicines could help.
If oral medications are not effective, a baclofen pump can deliver medication directly to the spinal fluid. This is known as intrathecal baclofen therapy (ITB). You can also use Botox injections for severe spasticity.
Even if you cannot move on your own, a caregiver or physical therapist can assist with gentle range-of-motion exercises to prevent worsening stiffness.
For nerve-related pain, neuropathic pain medications like Gabapentin, pregabalin, amitriptyline, and duloxetine might help. Sometimes, massage and hot compression also provide relief.
Wheelchair modifications and positioning devices can improve comfort.
Frequent infections like urinary tract infections (UTIs), pneumonia, or skin infections can worsen MS symptoms and overall health. Low-dose prophylactic antibiotics can help, and cranberry juice is very effective in preventing UTIs.
For pressure sores, you can use air-fluidized beds and advanced pressure relief cushions. Try to change your position every one to two hours. Use Betadine ointment on sores three to four times daily and maintain hygiene. If a daily bath is not possible, ask your caregiver for a sponge bath.
Fatigue is one of the toughest MS symptoms, and managing it requires multiple strategies. Amantadine, Modafinil, or Methylphenidate can help if MS-related fatigue is severe. Focus on small, nutrient-dense meals.
You can seek counseling or join MS support groups, including online groups, for emotional support. Try to divert your mind by listening to music or audiobooks, and practicing meditation and breathing exercises.
