Hi doctor,

Please advise considering metabolic surgery after a diabetes diagnosis last year, despite lifestyle changes. I am 36, and my current weight is 317 lbs at 5 feet 10 inches. My A1C improved from 9.2 % to 7.8 % with Metformin and dietary changes, but I still required increasing medication. Endocrinologists suggest that sleeve gastrectomy might achieve diabetes remission while benefiting from surgical benefits. Family history concerning - father and two uncles died from diabetes complications before 60. However, the psychiatrist worried about a history of depression and alcohol use disorder. Could surgery trigger addiction transfer or depression relapse? How would surgical recovery affect mental health medications? I need realistic expectations about diabetes improvement versus mental health risks.
