Hello doctor,
I urgently need your advice regarding metabolic surgery. I am 37 years old, 5'6", and weigh 312 pounds. Despite being on the maximum dose of metformin, Trulicity, and recently starting insulin (45 units daily), my diabetes remains uncontrolled. My most recent A1C is 11.8%. Additionally, my blood pressure consistently stays around 175/100 despite taking three different medications. I was recently diagnosed with moderate sleep apnea and fatty liver disease as well. My endocrinologist strongly recommended metabolic surgery, warning that I am at high risk for kidney failure if there is not a dramatic change soon.
However, I am terrified of surgical complications. My uncle underwent a gastric bypass and developed severe vitamin deficiencies that caused nerve damage, which has made me very fearful. I have tried every diet imaginable, worked closely with a nutritionist, and even completed a medically supervised liquid diet for three months, but I always regain the weight, often even more than before. With two young children depending on me, I am desperate to improve my health, but also scared of the risks involved with surgery.
Could you please advise:
