Does heavy bleeding with clots require medical attention?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I got my period 11 days ago, and my flow has been getting heavier. I am passing extremely large blood clots each time I urinate, and also experiencing heavy bleeding while asleep. What may be the problem?

Kindly advise.

Hello,

Welcome to icliniq.com.

I understand your concern.

Heavy menstrual bleeding with large blood clots and a prolonged period (lasting more than seven days) can be concerning. This could be caused by various factors, including hormonal imbalances such as PCOS (polycystic ovary syndrome), thyroid disorders, or perimenopause, which can lead to prolonged and excessive bleeding. Uterine fibroids or polyps, which are benign growths in the uterus, may also contribute to heavy bleeding and clot formation.

Conditions like endometriosis or adenomyosis can result in painful, heavy periods with clotting. Additionally, undiagnosed bleeding disorders could be responsible for excessive blood loss. If there is a possibility of pregnancy, a miscarriage should also be considered. Certain medications, particularly changes in birth control, can also impact menstrual cycles and cause abnormal bleeding.

To manage your symptoms, it is crucial to seek urgent medical attention if you experience weakness, dizziness, or lightheadedness, as these could be signs of anemia or excessive blood loss. Monitoring your symptoms by tracking the size of clots, duration, and volume of blood loss will help in diagnosis and treatment. Staying hydrated and taking iron supplements can prevent anemia, especially if you have previously taken them.

Consulting a doctor as soon as possible is advisable, as you may require tests such as an ultrasound, blood tests to check for anemia and clotting disorders, or hormone level assessments. If your bleeding is severe, such as soaking through a pad every hour or passing clots larger than a quarter, you should visit the emergency room immediately.

Thank you.
