Do treatment methods differ between private and government hospitals for multidrug-resistant tuberculosis?

Hi doctor,

My mother has been treated for multi drug resistant (MDR) tuberculosis (TB). Initially, the treatment was started at a private hospital. But the cost of her treatment was high. So I had to shift her to the government hospital for her treatment. They changed the regimen entirely because they started giving her Bedaquiline and five other drugs directly related to MDR TB. I wish to have a second opinion and understand whether the medicines provided are correct. She also has some side effects, including severe gastric problems, indigestion, and leg weakness. Please help. Thank you.

Welcome to icliniq.com. I understand your concern. This is an XDR (extensively drug-resistant) TB (tuberculosis) regimen. Is she taking any antacids? She has neuropathy because of Linezolid. We need to reduce or stop Linezolid and add Gabapentin or Pregabalin. Consult your specialist doctor, discuss with them, and take medications with their consent. I hope this information will help. Thanks for the reply. Is it okay to provide an extensively drug resistance (XDR) tuberculosis (TB) regimen for multidrug resistance (MDR) tuberculosis (TB)? Is it being done to reduce the chances of MDR turning into XDR? She takes Nexpro RD 40 once daily, in the morning before meals. Is there a way I can contact you personally? Thank you.

Welcome back to icliniq.com. I understand your concern. No, this is not like that. Her culture report might be showing resistance to Levofloxacin or Moxifloxacin. Or she might take Levoflox for over a month before this regimen. Which regimen did the private doctor put her on? I hope this information will help. Thank you.

Thanks for the reply. The doctor in the government hospital sent her sputum sample two days ago for testing resistance to Levofloxacin. Results are still awaited. So far, the reports show resistance to only Rifampicin and Isoniazid. Thank you.
