Hello doctor,
I masturbated after 30 days, and I found that when I masturbated my sperm was very thin, like a liquid, but was good in quantity. When I masturbated for the second time on the same day, the results were the same. Both times I felt pain in my penis region after erection. Why is this? Also, I have been masturbating for eight years. But in the past one to two months, I have witnessed a decrease in my sexual desires, feeling very tired after masturbating like a fever. What is happening to me?

Have you had any surgeries in the past? How many times a week would you say you masturbate on average? Do you have any trouble pulling back the foreskin of your penis? Do you have pain only when the penis is erect, or when the semen comes out as well? Are you sexually active or is this only during masturbation? Do you have trouble passing urine such as increased frequency, emergency, the feeling of incomplete emptying of the bladder, or a burning sensation? Is the pain ever in the testicle region? Do you sit for long periods in a single stretch? Have you had any scans such as an ultrasound or urine test? Please reply to the details and I will do my best to assist you.

Hello doctor,
-I am not a diabetic yet but I am prone to diabetes. My father has been diabetic for 30 years. I have experienced my blood sugar levels going up to 140 after having my lunch.
-I am not hypertensive but I have been in a state of stress for three to four years. I sometimes experience severe stress accompanied by frustration causing me not to sleep at night till 3 to 4 am.
-No I have not gone with any surgery.
-I do not masturbate regularly but when I do it I do it excessively like three to four times a day and then I will not masturbate for two to three days and then again. Sometimes I will not masturbate for many days. But in the past, I do it regularly.
-Yes since childhood I have had a problem with Phimosis, I am unable to pull back my foreskin when my penis is erect until I oil it or apply huge pressure.
- I have pain when the semen comes out as if my nerves are stretching in my pubic region. It is not something very new to me but has occurred to me in the past as well. I am also drinking very little water nowadays and not doing any physical activity for 10 days. I am just lying down on the bed the whole day doing my work. Also, I want to mention that I masturbate for long like 30 minutes on average by lying in bed, I will let the semen come up and then stop and then I will do this again and again repeating the cycle.
- I am not sexually active I only do masturbation.
- No I have no such issues with my urine passing. It has no problem. My urine passes smoothly.
-No I never felt pain in my testicles, I only experience pain inside my pubic region when semen comes out like my nerves are being pulled.
-Yes I sit for a long time like an hour on my bed until I become bored and then I go to the terrace and walk for some time.
- No I have not gone with any of these tests recently.

I understand your concern.
1. You will need surgery for the phimosis - there is no other way to get relief. The surgery is relatively simple and quick - you can contact your local urologist depending on your location.
2. I believe you are suffering from something called - Prostatitis. This is extremely common in your age group and especially common in men who do not ejaculate regularly and have prolonged sitting hours.
I suggest you do the following tests and get back to me with the results:
Ultrasound KUB with post-void residual urine
Uroflowmetry.
Semen for Culture and sensitivity- ejaculate and give the sample.
Serum Creatinine.
I will prescribe medications and some advice once I see the results.
