Why do lupus nephritis symptoms worsen despite treatment?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I was diagnosed with lupus nephritis last year, and I am concerned that my condition may be worsening. Recently, I have noticed increased swelling in my ankles and face, persistent fatigue, and foamier urine. My latest blood work indicates a decline in kidney function and higher protein levels in my urine.

I am currently taking Mycophenolate and Prednisone, but I am worried they may not be working effectively. Should we consider adjusting my medications or adding something new? Additionally, are there any dietary or lifestyle changes that could help support my kidney health? Please help.

Thank you.

Hi,

Welcome to icliniq.com.

I read your query and can understand your concern.

Your symptoms—worsening swelling, fatigue, foamy urine, and declining kidney function—suggest that your lupus nephritis (a kidney inflammation caused by systemic lupus erythematosus (SLE), an autoimmune disease) may be progressing. Since you are already on Mycophenolate and Prednisone, which are standard treatments, your doctor may need to adjust your regimen or introduce additional therapies. Here are some possible next steps:

Medication adjustments:

  1. Increase Mycophenolate dose – If you are on a lower dose, increasing it may provide better immunosuppressive effects.

  2. Switch to a stronger immunosuppressant – If Mycophenolate is insufficient, options such as Cyclophosphamide, Tacrolimus, or Rituximab could be considered.

  3. Add ACE (Angiotensin-converting enzyme) inhibitors or ARBs (Angiotensin II receptor blockers) – Medications like Lisinopril or Losartan can help reduce proteinuria and protect kidney function.

  4. Consider SGLT2 (sodium-glucose cotransporter-2) inhibitors – These medications (for example, Dapagliflozin) are typically used for diabetes but have been shown to slow kidney disease progression.

  5. Explore newer biologics – FDA (Food and Drug Administration)-approved options like Belimumab (Benlysta) or Voclosporin may be beneficial for lupus nephritis.

Kindly consult the doctor and take the medicines accordingly.

Diet and lifestyle adjustments:

  1. Limit sodium – Reducing sodium intake (less than 2g/day) can help manage blood pressure and swelling.

  2. Moderate protein intake – Too much protein may strain your kidneys, while too little can lead to muscle loss. A renal dietitian can help you find the right balance.

  3. Stay hydrated – Proper hydration is essential, but excessive fluid intake can be harmful if kidney function is declining.

  4. Monitor blood pressure – Keeping blood pressure below 130/80 mmHg is crucial to protecting kidney function.

  5. Avoid NSAIDs (nonsteroidal anti-inflammatory drugs) – Medications like Ibuprofen and Naproxen can further harm your kidneys. Kindly consult the doctor and take the medicines accordingly.

  6. Engage in low-impact exercise – Activities like walking or swimming can promote circulation without adding stress to your body.

Since your kidney function appears to be worsening, your rheumatologist (a doctor who specializes in autoimmune diseases) and nephrologist (a doctor who focuses on kidney health) may need to act quickly to adjust your treatment and prevent further damage. Be sure to discuss these concerns with them as soon as possible.

I hope this helps.

Please revert in case of further queries.

Thank you.
