Patient's Query
Hello doctor,
I am a 40-year-old woman diagnosed with lupus nephritis two years ago. Recently, I have been experiencing increased swelling in my legs and face, persistent fatigue, and joint stiffness. My latest lab tests showed worsening proteinuria, a rise in creatinine, and low complement levels. I am currently on Mycophenolate mofetil (CellCept), Prednisone, and angiotensin-converting enzyme inhibitors. Should we consider adjusting my treatment, and are there any lifestyle or dietary changes that could help slow kidney damage?
Please help.
Hello,
Welcome to icliniq.com.
I understand your concern.
Treatment Adjustments
Medication review: Re-evaluate your current medications, considering the worsening proteinuria, rising creatinine, and low complement levels.
Immunosuppressive therapy: Discuss potential adjustments to your Mycophenolate mofetil (CellCept) dosage or consider alternative immunosuppressants.
Corticosteroid management: Assess the need for Prednisone dosage adjustments to minimize side effects while maintaining disease control.
ACE inhibitor optimization: Ensure the ACE inhibitor is prescribed at an optimal dose to manage proteinuria and blood pressure.
Lifestyle and dietary changes
Sodium restriction: Limit sodium intake to reduce fluid retention and alleviate swelling.
Protein moderation: Follow a moderate-protein diet to reduce kidney strain.
Fluid management: Monitor and manage fluid intake to prevent dehydration and overload.
Blood pressure control: Regularly monitor blood pressure and adjust medications or lifestyle habits as needed.
Exercise and stress management: Engage in gentle exercises, like yoga or walking, and practice stress-reducing techniques, such as meditation or deep breathing.
Sleep and rest: Prioritize adequate sleep and rest to help manage fatigue.
I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.
