Patient's Query
Hello doctor,
I have lupus and was hospitalized last year for pneumococcal pneumonia. Since then, I have experienced irregular periods and now vaginal dryness, which is affecting my relationship. My rheumatologist suspects that I might be entering early menopause due to immune system dysregulation. I am wondering if the infection could have triggered these hormonal changes.
Should I consider hormone replacement therapy (HRT), or would that increase my risk given my history of autoimmune disease?
Kindly advise.
Hello,
Welcome to icliniq.com.
I understand your concern.
Managing lupus alongside reproductive and hormonal health can be especially challenging. Your infection and the stress it placed on your immune system may have triggered or accelerated hormonal changes, contributing to irregular periods. In people with lupus, immune dysregulation can sometimes affect ovarian function, leading to early or premature menopause. The vaginal dryness and menstrual irregularities you are experiencing could be signs of declining estrogen levels, which is not uncommon in autoimmune conditions, especially after a major illness or prolonged inflammation.
Regarding hormone replacement therapy (HRT), it is not automatically contraindicated in lupus, but it requires a cautious and personalized approach. If your lupus is stable and you do not have a history of blood clots or antiphospholipid syndrome, low-dose systemic HRT or localized vaginal estrogen therapy may be considered safe and effective to relieve symptoms such as dryness, hot flashes, and mood swings.
I suggest hormonal testing to assess if you are truly in early menopause; this includes FSH (follicle-stimulating hormone), estradiol, and AMH (anti-müllerian hormone) levels, as well as a pelvic exam to evaluate vaginal tissue health and rule out other causes of dryness. A coordinated plan involving your rheumatologist and gynecologist can help determine whether systemic or local estrogen therapy is appropriate based on your lupus status and personal risk factors. If you would like, I can assist in guiding you through this process with a customized plan that supports both your hormonal and autoimmune health while respecting your body’s unique sensitivities. You are not alone, and there are gentle, effective options available to help restore your comfort and confidence.
I hope you are satisfied with my answer.
