Does the presence of a lump underneath the skin on the face indicate folliculitis?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have had a lump underneath the skin on my face for over a month now. It is not itchy nor does it hurt. I know it is not a pimple because it does not have a head. Also, I feel pins and needles on my face along with the lump. Just starting now I have been having what I believe to be folliculitis. It is all over my body (my legs, arms, back, neck, head, etc.). It is extremely itchy and they look like tiny pimples all over. I do feel fatigued due to all of this.

Hello,

Welcome to icliniq.com.

Apply Clindamycin 1 percent lotion in the daytime for 20 days. Azac soap or Cetrimide soap for baths for one month. Apply Mupirocin cream twice daily for 15 days. Tablet Ciprofloxacin 500 mg once a day after breakfast for five days. Tablet Emanzen D (Diclofenac sodium and Serratiopeptidase) twice daily for three days (if pain). Maintain proper hygiene. Avoid scratching and rubbing of lesions. Wear loose comfortable thin cotton clothes. Avoid friction or occlusion. Do you have any history of fever, drug history, or allergies? Attach a clinical picture to assess the lesion so can guide you further.

Thank you.

Patient's Query

Hello doctor,

I am attaching a picture of the lesion. It is mainly underneath the skin. It is not painful. It does not itch. It is not soft or hard at the same time. It is in between.

Hello,

Welcome back to icliniq.com.

I have seen the attached picture (attachment removed to protect the patient's identity). It is frictional dermatitis with a secondary bacterial infection. Apply Mometasone cream at night for 10 days. Cetrimide soap for baths for one month. Apply Mupirocin cream in the daytime for 10 days. Apply Physiogel Hypoallergic lotion or Dermocalm lotion immediately after bath for 10 days. Take a tablet of Teczine-M (Levocetirizine and Montelukast) once at night after food for seven days. Avoid scratching and rubbing. Avoid friction or pressure. Wear loose comfortable thin cotton clothes. Maintain proper hygiene. Apply the cream with clean hands. Follow up after 10 days.

Thank you.
