PCOD (Polycystic ovaries) is a congenital condition. It is a genetically programmed condition you get from your parents. When you suddenly put on weight, this cholesterol is converted into estrogen hormone (if I may simulate- have not you seen obese men developing breast and in young obese girls, who develop periods at a younger age - they all have high levels of Estrogen hormone (female hormone)). In your body, too, when fat, which is a store for tomorrow, is more than usual, this is converted to the hormone that tips the balance between FSH (from the brain) and the ovarian hormone. As the name suggests, FSH (follicle-stimulating hormone) stimulates follicles from the ovary. Still, once the follicle is big enough to ovulate, the FSH is suppressed by the same estrogen hormone from the ovary. But when the same estrogen hormone is coming from fat, the brain is confused, and follicle growth stops early at a smaller follicle size and will not rupture. In the following scan, you will see all these follicles as polycystic ovaries.

This is usually associated with obesity, irregular cycles, no periods for a few months, and later heavy periods with clots and fleshy masses, spotting on and off, thyroid abnormality and prolactin problems indirectly.

You develop that extra pad of fat around the mid-segment of the body, especially the waist, thighs and breasts, with no fat beyond the elbow and knees. Excessive body hair growth, hair fall, a dark shade over the lower half of the face, acne, oily face skin, black skin over the back of the neck, inner thighs and under the surface of the breast. May have a family history of diabetes, especially with his father or his family.

This will not allow ovulation to occur at a time & so you cannot get pregnant till treated. It also increases your chances of early pregnancy abortions. But this has a solution.

Your weight has to be

Weight in pounds = 5 x BMI + (BMI divided by 5) x (Height in inches minus 60).

Your expected weight is 128 pounds to 132 pounds.

Once you reduce weight to the normal range, you will not need any medicine to get periods or to get pregnant, and pregnancy will be healthy.

To reduce weight -

  1. No fasting, junk food, fast food, red meat, or reduced snacks between meals.
  2. Have low-calorie food with high proteins.
  3. Regular exercise (especially for the waist and hip area), aerobics, yoga, brisk walking (swing your hands well), try to reduce eight pounds per month. Do not hurry on weight reduction; it will be challenging to maintain.

You will see changes as soon as you reduce at least eight pounds. It is difficult but will help get pregnant and reduce abortion chances.

Your age is nearing the upper limit of 35 years, but you still have four years in hand.

The result of treatment also depends on how low the count of sperm is. Keeping marriage of seven years, age of 31 years and semen factors- I suggest you look at investigations if it has been done.

  1. Husband semen analysis at least two, done four weeks apart, preferably in an infertility centre (examination done by an andrologist).
  2. Test wife's blood sugar thyroid profile.
  3. Ultrasound scans to evaluate the uterus, ovaries and adnexa. Confirm tubal patency. The tube connects the uterus and ovaries (the bridge where sperm meets the egg and forms a baby). This is the size of the hair follicle and cannot be seen on scans. Due to such a small opening, the tube can get blocked easily. So check with HSG ( X-ray with dye), SSG (Scan with dye) or laparoscopy. If all these become an upper limit, either go for IUI, the less costly option. If three cycles fail, IVF or ICSI can be thought of.
