Hello doctor,
I am a female vegan athlete struggling with low ferritin levels. I have been taking two iron pills each night for about six months now. My hemoglobin levels have increased as a result, but my ferritin level is still 7 mg/L. I was wondering if iron infusions would be a good option. I am looking for a quick fix because the fatigue is impacting me as an athlete, and I have been struggling with this problem for over two years now. I take B12 and calcium supplements, as well as a multivitamin. I have worked out the timings to optimize iron absorption, and my iron pills have vitamin C in them. I have tried every option besides iron infusions, not sure what is for low ferritin. Kindly suggest.

Usually, after six months of iron pill therapy, the ferritin level means the iron stores should come to the normal range. The iron pill increases iron storage first within three to four weeks. Your serum ferritin level is still low, so your hemoglobin level also should be checked. Repeating your serum ferritin estimation as low ferritin and normal hemoglobin after the iron pill is very unlikely. If your hemoglobin level is also low, then the underlying cause for iron deficiency should be ruled out by stool occult blood test, routine microscopic examinations, and less nutritional intake.
