Hello doctor,

My friend took 2.7 mg of Levonorgestrel tablet within two days of unprotected sex. She had taken 0.6 mg on the first day within 12 hours. When she took the second course of 0.6 mg after exactly 12 hours, she vomited the tablet immediately. So, she took 1.5 mg in 28 hours. Again, she took the second course of 0.6 mg in 34 hours as she wanted to complete the course. She is very worried about whether a heavy dosage can cause pregnancy. Please help.

My friend got her spotting recently and she started getting her usual periods immediately. Today is her third day of periods. There is bleeding with a few thick clots sometimes from day one. Does this prove that she is not pregnant? Please confirm. She had unprotected sex 30 days before her spotting and she took the pill 20 days after her intercourse. Should she take a pregnancy test even now once her periods are completed?

She seems to have her menstrual periods on time so the chance of pregnancy is almost nil. But, she can still do the urine pregnancy test after 10 days to confirm. It would be negative. By doing the test, she will be able to relax and her worry will be no more.

Also, can we know the reason for the blood clots which are getting released now which are not very common in her usual periods?

As she has taken extra doses of the pill, her endometrium became thick and is now bleeding causing more clots. It is usually not a cause for worry but if bleeding is more then I suggest she take a Tranexamic Acid tablet (Cellulose microcrystalline, Povidone (K 90), Croscarmellose sodium) twice a day for two to three days. Consult your specialist doctor, discuss with him or her and take the medicines with their consent.
