I have some kind of allergy in my inner thigh. What is that?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I am experiencing an allergic reaction in the inner thigh area near my anus. This has resulted in dry skin. It started as a small dot but has now spread on both sides of my anus and thighs. The itching is severe. Initially, I used a cream containing combination of Betamethasone, Gentamicin, Tolnaftate and Clioquinol, but now I am only using coconut oil. The affected area, as well as the surrounding skin, is becoming darker in color.

Kindly help me.

Hello,

Welcome to icliniq.com.

I read your query and understood your concern.

Firstly, I suggest you stop using a cream containing combination of Betamethasone, Gentamicin, Tolnaftate and Clioquinol as it contains steroids. It is possible that you have a case of tinea, and this cream you mentioned might actually exacerbate the infection. To provide an accurate diagnosis and recommend appropriate treatment, could you please share some clear photos of the affected area? This will help me in assessing the condition and providing suitable guidance. I hope this has helped you. Kindly follow up if you have more doubts.

Thank you.

Patient's Query

Hello doctor,

Thank you doctor for the prompt reply,

I have attached the photos of the affected areas. This condition is causing my skin tone to darken and resulting in persistent itching.

Kindly help.

Hello,

Welcome back to icliniq.com.

I read your query and understood your concern.

Based on the information you provided and the photos you have attached (attachment removed to protect patient's identity), It seems you have tinea cruris. It is a fungal infection that requires proper antifungal treatment. I suggest you follow the below mentioned treatment approach:

  1. Take a capsule containing Itraconazole (antifungal) 100 mg twice a day.
  2. Take a tablet containing Levocetirizine (antihistamine) and Montelukast once a day.
  3. Apply Luliconazole cream twice a day.

Continue this treatment regimen for two weeks. Tinea infections are becoming increasingly resistant nowadays, especially if you have also used steroids. Consequently, it might take longer for the infection to clear completely. Additionally, tinea infections have a tendency to recur. Therefore, please refrain from discontinuing the treatment until your doctor advises you to do so. I suggest you consult a specialist, talk with them and take the medications with their consent. I hope this has helped you. Kindly follow up if you have more doubts.

Thank you.

Patient's Query

Hello doctor,

Thank you for the reply.

I am also experiencing an issue with my biceps area, which appears to be affected by some form of allergy. Kindly help diagnose the underlying cause and treatment.

Please help.

Hello,

Welcome back to icliniq.com.

The photos that you have attached (attachment removed to protect the patient's identity) are not very clear. However, considering your existing tinea infection, it is possible that the affected area on your biceps could also be a tinea infection. I would like to inquire about the status of your tinea infection on the thighs. How is it progressing? I hope this has helped you. Kindly follow up with the requested information.

Thank you.
