Hello doctor,

I am a 48-year-old male with a history of borderline hyperglycemia under medication. I am repeatedly getting some multiple itchy points in my groin. There is a central main spot with eruption and whiteheads in one groin fold. The itching is more on the main spot, and some itching is there in the adjoining area and other groin fold. I had a similar type a couple of weeks back with pustules appearing on the opposing side of the testicular region. Then I took Oflox 400, Terbinafine 500, Tube Neosporin H tube for five days. It got relieved, but it is back with the same severity. There is some tenderness also.

I went through your history and photo (attachment removed to protect patient identity), you have developed tinea cruris. Tinea is a kind of superficial fungal infection. Diabetic patients are more prone to develop fungal infections. We need to start you antifungal treatment which will include Itraconazole tablets. I need to rule out any medicine interaction so I can decide the dose accordingly. Are you taking any medicine currently for any other illness? What medicine are you taking for diabetes? Take tablet Fexofenadine 180 mg one tablet daily at night for 15 days, Sertaconazole (Onabet) cream apply two times daily on the rash for three weeks. Please revert back with answers to my questions so I can suggest you the dose of Itraconazole also.

Take tablet Itraconazole 200 mg one tablet daily after breakfast for two weeks, Fexofenadine 180 mg one tablet daily at night for two weeks, Onabet cream, apply twice daily on the rash for three weeks, and tablet Zinfate (Zinc sulfate) one tablet daily at night for one month.
