I suffer from pain in my jaw when grinding with an upper Zirconia bridge (placed two years ago). I am experiencing pain in my ear. I am currently taking PropranoloI. I want to fix the pain while grinding and also the pain in my ear. Kindly help.

I have been taking Amlodipine 4 mg antihypertensive medication for only three months. I have been taking Propranolol for headaches for three years. I get ear pain only once or twice a week with or without chewing or eating food. I am getting ear fullness when bending over.

I can safely prescribe this tablet called Hifenac MR (Aceclofenac 100 milligrams, Chlorzoxazone 500 milligrams, and Paracetamol 325 milligrams) once a day for up to three to five days whenever your joint or ear pain aggravates. It will help.

As for your symptom of ear fullness when bending over, it is unlikely to be related to your zirconia bridge or bite. If you are bending and there is an ear issue, then it is a case to be investigated by an ear, nose, and throat (ENT) specialist in the clinic. I would recommend an in-clinic ENT check-up.

Also, you can ask your local dentist to trim a bit of the bridge where you bite down. Trimming the bridge can allow joint comfort and reduce your ear fullness.

Yes, the Zirconia bridge can be trimmed. Though the bridge may have a light risk of being slightly less strong than it was earlier (like when it was fixed), it is still more biocompatible with your teeth than a ceramic bridge.

Light trimming can be done with Zirconia, which can relieve a bit of joint and ear pain. Try asking your dentist to do that for you. Even after this, if your issue of ear fullness persists, then it needs to be addressed by an ENT specialist. The ear canals might need a check.

Thank you for your reply.

I had a regular doctor's checkup with light and that small type of camera. It is good to hear that zirconia is a good choice. I will give it a try and see if he can trim it. I have a bit of a problem with the lower left side tooth.

The dentist has advised me to take the cap off and perform a root canal. My previous dentist said to leave it for a year and see how it works. What do you think? It is not painful, but it does have a small cavity in there somewhere.

I agree with this treatment. A cap without a root canal is ideal when you do not have any sensitivity or pain in that tooth. Do you have a sensitivity or not?

From the images that you have shared (attachments removed to protect the patient's identity), around the bottom left tooth, where the cap is displaced, I can see bone loss in that region.

  1. Can you tell me when you last got a dental cleaning done?
  2. Did they do dental scaling for you?

May I know where you can see the bone loss occurring at the root end? This cap was replaced one year ago. As of now, I have no pain or sensitivity to hot or cold stuff, but I can experience a bit of galvanic shock or oral galvanism. Yes, they cleaned my teeth about three months ago.

The bone is receding between the first and second molars. I feel you should go with the new cap, and if you are not having any issues like pain or sensitivity in the lower left last tooth, then there is no need for a root canal treatment as such. Just go for a new zirconia cap. Undergoing dental scaling every three to six months is good for preventing vertical bone loss.

Get the zirconia trimming done soon, and also, please go for an ENT check-up before you proceed with your dental treatment. You will know the findings, if any.
