Hello doctor,

I am 45 years old female, suffering from too much itching for one year at the place on my left leg. When I consulted a doctor(dermatology) he recommended DiprovatE lotion and levocetirizine tablets. When I used them it worked temporarily. Please suggest the curing process.

I read your query and understand your concern. You are having Lichen simplex chronicus (LSC). It arises because of repetitive scratching or rubbing there is the thickening of the skin with variable flakes. Lichen simplex chronicus usually describe stable itchy thickened one or more areas; however, thickening of the skin occurs in any location that the patient can reach, like the nape of the neck, extensor forearms and elbows, scrotum, upper medial thighs, knees, lower legs, and ankles. In your case, it involves extensor aspects of the ankles. I suggest you,

1. Apply Xerina cream or moisture cream wait for 15 minutes then apply topical steroids with salicylic acid (Propysalic NF lotion occlusive dressing will be a more helpful method of occlusion, after application of Propysalic lotion above this put a flat polythene cover then cotton bandage leave it for the whole night for 2-3 days, occlusion not need in the day time ) should be applied twice daily for we two to four weeks.

2. One tablet of Levocetirizine once daily for a similar duration, or you can try an injection of Kenocort (triamcinolone acetonide)10mg/ml 0.1 ml/cm two intralesional at the three weeks interval should be given by a dermatologist.
