Hello doctor,

I am a 29-year-old male. I recently realized that my scrotum is itching and there is pimple-like stuff that is bleeding a little. There are reddish-black spots on my scrotum, but it is not the area that bleeds. The area that bleeds looks like a ring cut.

Please help.

I understand your concern. The black, reddish papules may be angiokeratoma which bleeds when you scratch that, but that is not the cause of itching as such. I am prescribing you medications for itching and ring-shaped erosions. Kindly consult your doctor to discuss the suggestions and take the treatment with consent.

1. Tablet Fexofenadine 180 mg twice a day.

2. Tablet Ivermectin 12 mg today and after a week.

3. Cream Flutibact (Fluticasone Propionate, Mupirocin) cream twice a day for a week.

The common cause of itching in the scrotal area can be scabies, scrotal dermatitis, or tinea which is a fungal infection. It does not look like tinea, and I have prescribed treatment for the rest of the two conditions. The erosions are secondary to the itching part. Wash the area with normal soap and water, before applying the cream and avoid scratching too much. Wash your clothes in warm water.
