Hi doctor,

The tip of my penis has itching and inflammation. I am diabetic, and hence this may have occurred. I have been through this twice in the last 10 years. Once sugar comes into control, it subsides.

While I am attempting to control blood sugar, please suggest a cream that can be applied to the foreskin. The foreskin splits when I try to retract it and thus causing pain and itching, and there is a cheese-like substance under the foreskin. When I try to retract to clean this, the foreskin splits, causing pain. Currently, I am taking Triglynase two times a day. Please advise.

I understand your concern.

It seems you have balanitis. Balanitis is inflammation of the glans penis. Balanitis involving the foreskin and prepuce is termed balanoposthitis. Predisposing factors include poor hygiene, overwashing, over-the-counter medications, and non-retraction of the foreskin. It may be associated with diabetes and candidal fungal infections. I recommend you consult a specialist and get a blood sugar KOH mount (Potassium hydroxide) for candidiasis.

Also, I would suggest you gently retract the foreskin daily and soak it in warm water to clean the penis and foreskin. Then, you can use a combination of Fusidic acid and Clotrimazole cream twice daily for one month. Finally, if balanitis does not respond to topical therapy, you can take the tablet Fluconazole 150 mg.
