Hi doctor,

I am very tired and not just usually tired, I would call it being sleepy because it is even difficult to keep my eyes open all the time. I cannot express enough how tired I am. I have a history of sleep disorders, and I thought I got over them. They were never quite like this. They did not include such insane tiredness, but it was more of a sleep onset problem and just feeling crazy. Now in this new version, over a month ago, I got less and less sleep each night, and I was always tired. I feel like I am more tired every day. My issue is not necessarily the onset of sleep but what happens after the onset. I cannot stay asleep for more than two hours but wake up constantly throughout the night, and the sleep quality is incredibly bad because the overwhelming tiredness takes over as soon as I open my eyes. I usually feel myself just experiencing hypnagogic hallucinations, and that is as deep as the sleep gets. Then I attempt to repeat this a few times a night. Sometimes it has felt like I have actually been asleep for like six hours total with all the waking up, but still, the extreme tiredness or sleepiness persists. My blood test results were normal. I do not have sleep apnea, at least according to my smartwatch, which also detects almost no deep sleep. I also do not have any medications, I am trying to be as physically active as I can be, and I eat very healthily. I am at a point where I am starting to think if I should worry about neurological diseases like fatal insomnia because of how unexplained this is. Despite this, I try to remain calm, and I do not have anxiety or stress. I meditate and do not use a screen before bed. I would need your professional opinion based on these sleep symptoms if you think I should worry. Thank you very much.

I understand your concern.

Your symptoms are compatible with primary insomnia. For this reason, I suggest starting Melatonin 5 to 10 mg before sleep. If this does not help, I would suggest starting a low dose of Mirtazapine 7.5 mg before sleep and increasing the dose by 7.5 mg each week until reaching the desired effect (a normal sleep). This drug can help regulate your sleep cycle if taken for a couple of months. I would also suggest checking thyroid hormone levels for possible thyroid gland dysfunction, which could lead to this situation.
