Why is there severe toothache with swollen face even after taking antibiotic?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My tooth started hurting about a month and a half ago, just one night of hurting badly. Then went away. About weeks later started hurting again but nothing too bad. I just dealt with it. Now, again I have severe pain. I went to ER, said it was abscessed tooth and gave me Amoxicillin. The pain was still very bad so went back to the doctor. Three days later they prescribed me Clindamycin. Started taking them this morning. The pain is very bad and now my face is starting to get swollen. My whole face hurts, hurts to talk or really do anything. Should I go back to the ER or wait and see if this new antibiotic works? Please help me.

Hi,

Welcome to icliniq.com.

I have gone through your question and I can understand your concern. I think you are having an acute periapical infection with involvement of buccal and mandibular space infection. If the swelling is increasing and there is burning pain you may need another antibiotic with Clindamycin. For that upload a picture so that I can provide you with the best possible treatment plan. Make sure you do not have any airway obstruction, that can be dangerous.

The Probable causes

Periapical infection due to the cavity.

Investigations to be done

IOPAR X-ray.

Differential diagnosis

Acute apical periodontitis.

Probable diagnosis

Buccal space infection.
