Hi doctor,

I had my first intercourse more than a week ago (just when my period ended). The next night, I took an I-pill. After three days, I started experiencing frequent and painful urination with a little blood in my urine. It has been a week since then, and the pain still persists while peeing, and the amount of blood coming out has increased. It is like a burning sensation while urinating, and blood is coming out a lot now. What is happening to me, and what should I do?

I understand your concern.

We have two situations at hand. Let me explain.

1. A woman with normal cycles usually ovulates around the 14th day of the cycle, and the egg stays alive for 24 hours only. Sperms will stay alive for 72 hours only. So for a woman with 28 days cycle, the chances of pregnancy are highest from day 11 to 18 of the period. The first day of the period is the first day of bleeding. I mean to say that your chances of pregnancy were nearly zero around the time periods, so there was no need for I-pill. Now, since you have had I-pill, this is nothing but a high dose of the hormone progesterone, which is produced in the second half of the period. This single high dose suddenly increases the inner lining of the uterus and makes it unfavorable for the possible pregnancy to sit, but within seven to ten days, this thickness starts falling off as thick, dark blood, with mild pain. This could be the blood you are seeing. Just wait for a few days. This bleeding would subside. But I would suggest you not have I-pill frequently or after every sex, as this will disturb your hormonal balance and periods. Once disturbed, you will have no track of ovulation and periods. Also, your weight is towards the upper limit. Your expected weight is 114 to 126 pounds. This additional weight will be converted to hormones and can further impair hormones, ovulation, and periods. Do try to cut down on weight. If expecting sex more frequently, have cyclical combined oral contraceptive pills. If not, use condoms.

2. Being the first sexual exposure, you would have torn the hymen and also mild abrasions in the walls of the vagina. This would also cause minimal bleeding. So what you feel as burning during passing urine can be a vaginal infection. Also, after you have sex, it is good to pass urine immediately. As the semen will enter the urinary bladder too and it can lead to urinary infection. Hope your partner has no infections. For this, have a tablet of Norflox (Norfloxacin and Lactobacillus) 400 mg twice a day for five days. Vaginal suppositories like Clingen forte (Clindamycin, Clotrimazole, and Tinidazole), Cansoft (Clotrimazole) 100 mg pessary, or Candid-V (Clotrimazole) can be kept in the vagina at night for five nights. Have a lot of water and keep passing urine frequently. Refrain from sex till you are fully okay.
