Does HMPV cause long-term lung damage in COPD patients?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My 67-year-old father, who has a history of mild COPD (congestive obstructive pulmonary disease), was recently diagnosed with human metapneumovirus after presenting with a persistent cough, low-grade fever, and difficulty breathing. He was started on bronchodilators and supportive care, but his symptoms, particularly the shortness of breath, seem to have worsened in the past few days. His oxygen saturation has occasionally dropped to 89 percent during exertion, and his latest bloodwork showed elevated CRP (C-reactive protein) and white blood cell counts. A chest X-ray revealed mild bronchial thickening but no consolidation.

Could this progression indicate a secondary bacterial infection, or is it likely due to exacerbation of his COPD caused by the virus? Should we consider starting antibiotics, or would corticosteroids or nebulized treatments be more effective in managing his condition? Additionally, we are worried about the potential for long-term lung damage or recurring respiratory issues due to this infection. Are there specific tests, such as a CT (computed tomography) scan or pulmonary function tests, that would help determine the severity of the impact on his lungs?

Please help.

Hello,

Welcome to icliniq.com.

I understand your concern.

As per history looks more like an exacerbation of COPD (congestive obstructive pulmonary disease) with infection.

I would like to know the following listed below:

  1. What is his WBC (white blood cell) count?

  2. Differential CRP (C-reactive protein) values?

  3. Does he have any other comorbid conditions like sugar, BP (blood pressure), or thyroid?

  4. Any addiction?

  5. It is essential to plan for PFT (pulmonary function test).

I hope this has helped you.

Please feel free to reach me again, in case of further queries.

Thank you.
