How to effectively manage high blood pressure?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have been suffering from high blood pressure for the past 10 years. I take medicine twice a day, but my blood pressure remains the same at 150/100 mmHg. I take Metoprolol succinate (at night) and Chlorthalidone (during the day). I am a 48-year-old female, and my cholesterol is slightly high, so I take Rosuvastatin. However, my ECG is normal. I also have high TSH levels and take Levothyroxine daily.

Please help.

Hello,

Welcome to icliniq.com.

Your blood pressure (BP) is not controlled with the beta-blocker (Metoprolol succinate) and diuretic (Chlorthalidone). I advise you to undergo a renal function test and a renal Doppler. The cause of your high BP may be related to renal pathology, which should be ruled out. If both of these tests are normal, some changes to your medication will be necessary. For now, you can add Amlodipine 5 mg once daily. Consult your specialist doctor, discuss with him or her, and take the medicines with take the medicines with their consent.

I hope this helps you.

Thank you.

The Probable causes

The probable cause is secondary hypertension (HTN).

Investigations to be done

Investigations to be done include renal function test (RFT), renal color Doppler, blood sugar tests, and electrolytes.

Differential diagnosis

The differential diagnosis is essential hypertension and secondary hypertension.

Probable diagnosis

The probable diagnoses are hypertension, hypothyroidism, and hypercholesterolemia.

Preventive measures

Preventive measures include exercise, following a low-salt diet, and reducing stress, if any.
