Hi doctor,

I have an inflammation in my groin area. It becomes very hot and sweaty, especially in my anus that makes me think of the need to defecate, but mostly I do not. At work, I cannot sit or have a meeting for a long time without feeling uncomfortable and needing to go to the toilet, which is a false alarm most of the time. I do vape, which makes me slightly bloated, but I also get bloated after eating food. I went back to smoking cigarettes to make sure vape is not causing it because two things happened when I started having this issue. First, I had very painful constipation, which was due to carrying my furniture to the other house by myself, and after that was playing football and got dehydrated, which caused a hard stool that injured me. The next thing is that I started vaping and quit smoking cigarettes. I stopped vaping for two days, but it continued, and I do not have any bumps in my groin area. Another issue is causing this problem because my friends vape and smoke, and none have this problem. And the constipation injury should be over by now because it is almost one year now since this issue began. Sometimes I take the antidiarrhea medication with Paracetamol, but that is not enough. I used hemorrhoids cream through self-diagnosis. I went to two clinics, and they gave an antidiarrhea prescription and charcoal medicine to clean the stomach, but it did not help. I am 31 years old. Please help me.

According to your symptoms, I think you are suffering from hemorrhoids. You should take care of your diet primarily. Eat more leafy vegetables, cut out nonvegetarian, spicy, and oily foods. Drink plenty of water. Quit smoking. You can use the hemorrhoid cream. You should sit in warm water mixed with salt daily for around 20 minutes (sitz bath). If you find blood, then you must take iron supplements daily. If you feel your problem is getting worse and not healing, then you must see a surgeon because surgery is the only option if it does not heal. You can also take tablets for gastritis. I suggest Omeprazole 20 mg tablet twice daily half an hour before food. You will get well soon. Thank you.
