Hello doctor,

A severe hemophilia B patient here. I just developed high-titer inhibitors. I had a breakthrough in ICH despite BPA therapy. GCS is dropping and I have a neurosurgeon on standby. Factor IX levels are undetectable. I am considering Factor IX products but there are cost issues.

This is a life-threatening situation. On the basis of your symptoms and laboratory reports, urgent intervention is required.

Here are a few things to know:

  1. Factor IX (FIX) administration increases the risk of anaphylaxis or allergic reaction in inhibitor patients. Close monitoring and premedication (steroids or antihistamines) are needed.

  2. If the current dose and frequency of FEIBA (factor eight inhibitor bypass activity) or NovoSeven (coagulation factor VIIIa, recombinant) have not already been maximized, increasing them might help control the bleeding. However, since you have already had a breakthrough intracranial hemorrhage (ICH) despite being on bypassing agents (BPAs), these treatments may not be effective enough in this case.

  3. Factor IX-containing products can be used, but cost is a major barrier. You can try to access any patient assistance programs or hospital emergency supplies.

  4. As your GCS (Glasgow coma scale) is dropping you need urgent evacuation of hematoma, EVD (external ventricular drainage) placement, or decompression.

  5. As per the cost, you can ask for assistance from hospital charity or emergency access programs.

  6. You can contact pharmaceutical companies directly. Your medicine cost will be less if you buy from them directly.

  7. Hemophilia foundations may also offer urgent financial aid.

    You need urgent medical care and may also need ICU (intensive care unit) care. Decide ASAP (as soon as possible), and try collecting funds from the sources mentioned above.
