Hi doctor,
I have been having frequent (almost constant) headaches and they have gotten worse over the past week or so, and I have also had dizziness, blurred vision, balance problems (I keep falling over for no reason), nausea, and just yesterday I started talking with a stutter (that is very unlike me, normally I talk pretty fast and I have never had a stutter before).

7. How is your day today?
8. How is your sleep?
9. Have you noticed any trigger factors for your headache?
10. Have you ever undergone any investigations for this headache ? If yes, what were the test results?
11. Are you taking any medications for it? If yes, what are the medicines and their dosage, and what has been the effect of those medicines?
