Hello doctor,
I want to consult about my aunt, who is 78 years old. She has been experiencing greenish-colored urine and compromised hygiene for nearly two years and does not know what the issue is. She says it began at the time of the COVID-19 vaccinations. I am aware that there can be immune system complications beyond simple blood count differentials in some people. There may be a problem in the urinary system with bacteria that have not responded to antibiotics. She has been prescribed a few different antibiotics. She has tried Detrunorm XL, but I cannot remember the others. She takes Ranitidine regularly for incontinence. She has been fit and well for many years with no other complaints. Also, she has been placed on Levothyroxine and is experiencing side effects. I do not think her TSH is far beyond her age group's normal range. Can you please advise us on what the issue may be and if there are steps we can take to resolve it?

From what I can understand, it seems that your aunt is suffering from a bad UTI (urinary tract infection), which needs treatment. Please help me with some more details so that I can assist you better: Is she diabetic? Does she have difficulty in passing urine, like a burning sensation, pain, urgency, or a feeling of incomplete emptying? Has her doctor done a urine routine and culture test? The report you have attached (attachments removed to protect the patient's identity) is a blood workup and is pretty much within the normal range. Does she have any pain in her loins? Does she have any vaginal discharge? When was her last course of antibiotics?
I would advise you to get a urine routine and microscopy, urine culture, and CBC (complete blood count). The tablet Detrunorm XL (Propiverine) is for urgent symptoms and will not treat infections. I suggest she take Cranberry and D-mannose tablets twice daily, and Saccharomyces boulardii is to be taken twice daily. Consult a specialist doctor, discuss with them, and take medications with their consent.

She is not diabetic. She has no difficulty passing urine. Urine microscopy and culture tests have been done. She does not remember the name, but she took Azithromycin 500 mg for five days. She takes Pantoprazole 20 mg before breakfast, Pravastatin 10 mg, and Dalmane 15mg before bed. She used the Ovestin cream due to irritation. She does not know if she was using it correctly. She keeps telling me that her womb is pressing against the bladder. There has been no scan, but this may be common in females her age. I do not know. We will pick up the natural supplements as suggested and begin immediately, along with a glass of lemonade per day.

I cannot see any reports for a urine test. So my reason for switching to Mirabegron in the morning and stopping Detrunorm (Propiverine) is that it works better in my experience. She should continue to use the tablet Levothyroxine. The vaginal cream should be used every day at bedtime. It helps prevent UTIs (urinary tract infections) in post-menopausal women. Even if the uterus has prolapsed and pressed on her bladder, it is not the reason for UTIs. It will just cause urgency and increased frequency. But that is something I cannot comment upon without an examination. Also, consider taking the supplements I have suggested for at least two to three months.
