Hi doctor,

I am a bit anxious about going to the dentist as it has been three years since my last cleaning and check-up. The only issue I have, and have had for many years, is bleeding of the gums when I brush. I have noticed, over the past few days, that I have been flossing and brushing more religiously. There is still some bleeding, but it is gradually getting better. My gums, in general, are not painful to touch but are a little sore above my back molars. There are a few spots between my teeth that are dark but there are no big black triangles along the gum-line. I have not noticed any dull or sharp aching tooth pain other than that which comes and goes with sinus and allergy issues. I would like to know if I should be concerned about my gingivitis advancing to periodontitis and what are the telltale signs of more advanced gum disease. Please help.

When gingivitis (inflammation of the gums) proceeds with alveolar bone loss, it is considered periodontitis. The extent of bone loss can be identified by X-ray examination. Further, bone loss leaves signs like gum pocketing around the tooth (which only the dentist can identify after careful clinical examination), and receding of the gums due to lost alveolar bone support. In more advanced cases, loosening of the teeth can occur. You must go to the dentist for a clinical and radiographic examination.
