Hi doctor,

I have gestational diabetes, and my blood sugar level on fasting was 111 after 10 units Insulin. And it was 200 after breakfast. My doctor is on leave. So please suggest me what to do.

I understand your concern. I have reviewed the reports (attachment removed to protect the patient's identity). Only one blood sugar reading shows a high blood sugar level, and fasting blood sugar is normal. So it is likely that the cause of your high blood sugar is your breakfast. Instead of increasing the Insulin dose, which can cause hypoglycemia (a condition in which your blood sugar or glucose level is lower than the standard range) attacks, it is better to reduce the glycemic index of your breakfast, which means low carbohydrate content. Secondly, the type of Insulin you are taking is outdated, and according to international guidelines for diabetes in pregnancy, it is not prescribed for pregnant women anymore. Take three small Insulin Apart (NovoLog) 2 units each for breakfast, lunch, and dinner. Insulin Levemir (Detemir) 2 units at bedtime are recommended to control blood sugar and avoid hypoglycemia attacks.

Thank you for the reply. Please advise if I should take these Levemir Insulin two units 15 minutes before breakfast, 15 minutes before lunch, and 15 minutes before dinner.

I understand your concern. No. I advised you to take Insulin Aspart (NovoLog) 2 units before or with breakfast, 2 units before or with lunch, and 2 units before/with dinner. In addition to these, take Insulin Levemir (Detemir) 2 units before bedtime only.

Same symptoms don't mean you have the same problem. Consult a doctor now!
