Hello doctor,

There is some problem with my toenails. My big toe nail has turned brownish and yellowish in color on the bottom and sides. Also, that part of the nail seems to be dead, and I can detach it from the skin beneath it. I play sports regularly, and the shoes are tight and sometimes hurt my feet and big toenails. I think that might be causing this problem, or can this be caused due to fungal infection? There is no pain, and I have had this for a few months. What is the problem, and is there a need to be concerned? Kindly help.

I reviewed the pictures (attachment removed to protect the patient's identity) and this onychomycosis, a fungal nail infection. I would suggest the following medicines:

  1. Anti-fungal (Terbinafine or Itraconazole) and topical anti-fungal nail lacquer (Ciclopirox) for at least three to six months, depending on the response.
  2. Also, consult a dermatologist in person for the prescription.The doctor may also send the nail clipping for KOH examination and fungal culture to know the type of fungus causing it and to decide the best anti-fungal medicine that works for your nail fungus. Since this is an infection, it can spread to other nails or other body parts. Therefore, I would suggest getting the treatment as early as possible.
