Hi doctor,
I am a healthy 36-year-old male. I have had severe itching since last week, especially in my right leg, calf area, hamstring area, and front part of the thigh. I noticed some redness also in that area. I am not sure if the redness is from the itching or if the redness caused the itching. I visited a doctor who prescribed me Triamcinolone acetonide 1mg/g cream. I have used it for two days but still have itching. I have attached some pictures. Does it look like some allergy or infection? Kindly help me with a solution.

It is a fungal infection with contact irritant dermatitis. I suggest you apply two percent Ketoconazole lotion 20 minutes before going to bath for 20 days. Use Ketoconazole soap for a bath for 2 months. Avoid scratching and rubbing lesions as they may spread. I suggest you take tablet Teczine-M (Levocetirizine and Montelukast) once at night after food for seven days and tablet Loratadine 10 mg after breakfast for 10 days. Hope this will help you. Sorry to ask again, but I believe fungal infection has a ring-like structure. I am so confused now because the doctor I visited said there is no infection. Also, can steroids worsen fungal infections? So is there an infection on the entire leg? Kindly help me.

Thank you for your response. M.furfur (Malassezia furfur) species cause furfuraceous scales rather than the typical ring-like variant caused by dermatophytes. Therefore, I would suggest stopping steroids. Hope this helped you.

I will stop taking steroids. Can I get a prescription? I will try to get some anti-fungal until I see a doctor on Monday. I will get Miconazole powder or Clotrimazole powder otc, which one should I use? Also, I have been touching my face and eyes after scratching. Is there a risk it can spread to the face or eyes? Kindly help.

No, you can use anti-fungal in lotion and soap form. However, if there is excess sweating, do not use powder form. Hope this helped you.
