Hello doctor,
I had a fungal infection around the thigh area close to the genitals and skin irritation around the neck and finger knuckles. I got treated earlier with paraffin application, Fluconazole tablet, and skin cream. Since it is winter now, I again experience those symptoms. I want to know what mild antibiotic or antifungal tablet intake with a skin cream can help me overcome this.

After going through the history and images (attachments removed to protect the patient's identity) attached, I suppose it is Tinea cruris (affecting folds and unexposed areas) with Tinea corporis (affecting the trunk and other body parts).
Tinea cruris is a fungal infection most often seen on the folds and private areas where exposure is less. They appear due to the stagnation of sweat and moisture. Private areas, groin, and buttock areas are major skin fold areas that would be affected. Underarms, under the abdomen, and folds of arms and legs, are other areas to be affected. They may also form due to decreased immunity, spread from an infected person, hygiene issues, climate changes, and hormonal changes. They appear recurrently.
Due to external and internal triggers, as mentioned above, Tinea corporis is similar but affects exposed areas on the body instead of folds. They present with itching, scaly lesions, sometimes oozing and foul-smelling secretion, and occasionally pain and redness. Itching is the most common feature, which may lead to discomfort throughout the day, especially when sweating. Apart from the same, there are factors to prevent recurrences of the same. Tinea infections are usually on and off and recur due to multiple factors as listed above, due to which its prevention may be done with the below advice.
