Hello doctor,
I have noticed the presence of red little dots on my penis that disappeared for a while but then reappeared. This issue started approximately a month ago, initially appearing in one area on the head of my penis. However, I made the mistake of washing it with soap, which caused the dots to spread.
I began using moisturizer, which seemed to help them go away, but they returned once I stopped using it. Additionally, I have noticed a foul smell when I pull back the foreskin, which only started last week. I have taken extra care to clean my penis thoroughly, but the dots seem to appear after having unprotected sex with my girlfriend.
The head of my penis often appears red or pinkish, and then the dots reappear. These dots do not cause any pain, itchiness, or discomfort.

Thank you for the reply.
I do not experience any penile discharge or painful urination. There is no pain or swelling in the testicle area. I noticed the appearance of these lesions on the following day after having intercourse.
These lesions are only present in the specific area shown in the picture, and there are no similar lesions elsewhere on my body.

Based on the information you provided and the attachments (attachments removed to protect the patient's identity), there is no evidence of any sexually transmitted infection (STI) causing these symptoms. Based on the information provided, it is likely that the sensitivity of the glans penis is the underlying reason.
Excessive hygiene is the most common cause of irritation on the glans. I suggest you wash the genitals with lukewarm or warm water once daily. And also, avoid changing soaps frequently and maintain adequate hygiene without excessive cleaning.
Gently cleaning the shaft of the penis with pulled foreskin using a suitable soap is sufficient to prevent the accumulation of foul-smelling substances. I suggest you continue applying moisturizing cream as it has been beneficial so far. In addition, I suggest you apply Hydrocortisone 1 % cream once a day for one week, only on the affected area. Please consult your doctor and then take the medications.
Monitor the response and discontinue use if any sensitivity reaction or other complaints occur. For now, I suggest you use only the moisturizing cream. I will follow up after five days to assess the response.
