Hello doctor,

I have had fever, chills, low-back pain, difficulty urinating, painful urination, watery semen, yellowing semen, no force behind ejaculation, and low semen volume for one week. The semen is still slightly yellow, has no odd odor, is very watery, and has no force behind ejaculation. Kindly suggest.

I understand your concern.

I suggest you initiate a diabetic diet, semen consistency also gets affected by it, also with increased blood sugar levels you're more susceptible to frequent UTI (urinary tract infection). Also, your lipid profile is deranged, you need to control oily foods in your diet. For 15 days follow the diet change, and also initiate brisk walking early in the morning. Get yourself investigated again for the same, if the reports are improving there's no need for anti-diabetic therapy, if it remains the same, you will need to add antidiabetic therapy to your medication. Monitor as follows:

An HbA1c level of 6.5 % or higher; the test should be performed in a laboratory using a method that is certified by the National Glycohemoglobin Standardization Program (NGSP) and standardized or traceable to the Diabetes Control and Complications Trial (DCCT) reference assay. Or

A fasting plasma glucose (FPG) level of 126 mg/dL (7.0 mmol/L) or higher; fasting is defined as no caloric intake for at least eight hours. Or

A two-hour plasma glucose level of 200 mg/dL (11.1 mmol/L) or higher during a 75-g oral glucose tolerance test (OGTT). Or

Random plasma glucose of 200 mg/dL (11.1 mmol/L) or higher in a patient with classic symptoms of hyperglycemia (ie, polyuria, polydipsia, polyphagia, weight loss) or hyperglycemic crisis.
